Back Pain in Sports Players – Can it Be Sacro-Iliac Joint Dysfunction?

Many of the football players treated by Milton Chiropractic Clinic present to our clinic with Sacro-Iliac pain. Often, pain arising from this joint is misdiagnosed as being a “slipped disc”, since the pain caused by the Sacro-Illiac joint can refer into the leg. But in contrast to a slipped disc, the pain rarely radiates below the knee. Typical signs and symptoms of Sacro-Iliac Pain are a dull ache, located at the base of the spine which intensifies with sitting, bending, lifting and standing up from a seated position. Sometimes the pain can refer to the groin, buttock or the back of the thigh.

The Sacro-Iliac joints are located at the back and bottom of the spine and consists of Ilium bones and the Sacrum. The joint surfaces are covered with articular cartilage and the joints are reinforced by thick, strong ligaments, which give the joint great stability.

Sacro-Iliac Joint pain can have several causes, but the one of the most commonest ones is when a person lands on one buttock, during a fall. Something not uncommon in football! A leg length discrepancy might also create Sacro-Iliac pain.

Some inflammatory disorders will also produce pain in the Sacro Iliac joint. The general term for these disorders is: ‘spondyloarthropathies’, which simply means a disease that affects the spine. Examples of these conditions are: ankylosing spondylitis, psoriatic arthritis, inflammatory bowel disease and Reiter’s syndrome. The interesting thing about these conditions for the medical staff is is that they occur especially in young men, aged between 16 and 35. These conditions can only be treated with medication, and a referral to a Consultant Rheumatologist is required.

Treatment: What Can You Do?

• Consult a specialist

• Rest from aggravating activities

• Wear some support, like a brace or a belt

• Do light exercises, preferably in a pool

• Use corrective insoles

Chiropractic Care is very effective where normal Sacro Iliac joint movement is ‘blocked’ and needs to be restored. For Chiropractors – Whiplash and Personal Injury Analysis – Penning’s Method

This analysis method is by far one of the most useful for determination of loss of range of motion that I have found. This is a great way to show pre/post x-ray changes in a quantifiable way, and to show how the care you provided helped your patients by improving ligament stability in the cervical spine.

To establish hypomobility or hypermobility of vertebral segments in the sagittal plane, Penning’s Method is a one of your greatest methods. This is done with functional examination of the cervical spine in flexion and extension, using radiographic analysis.

If there is a loss of range of motion present, several factors may be found in your patient. You as the attending Cape Girardeau Chiropractor will need to understand these variable factors in order to ensure that the “validity of assessment” criteria have been met according to standards. Doing so will also best help your patient should a personal injury case enter the court room and you find yourself on the witness stand.

The intention of this article is to briefly explain and help Cape Girardeau Chiropractors use this method of functional radiographic diagnosis of the cervical spine during flexion/extension. This will hopefully allow you to better serve your patients and to have solid pre/post analysis. There has been much research done on this topic by Dr Liberti, D.C., as well as many other Doctors and Physicians, such as Dr. Pennings himself.

Completion of Penning’s Method

Penning’s Method is known by many to be the most widely accepted and utilized method for determining flexion/extension motion. To start the extension film is superimposed on the flexion film, with the C7 vertebrae matching perfectly. Next a line is drawn along one of the edges of the extension film, onto the flexion film. You then do the same with C6. A second line is then drawn. You can now measure an angle between these two lines and this establishes the degree of motion between the C6 and C7 vertebra. You follow this sequence for the rest of the cervical spine to determine the degree of motion between all of the vertebrae. The average values of segmental motion are written about by Dr. Penning: Functional Pathology of the Cervical Spine, 1968 pg 1-25. These values can be compared to determine the amount of dysfunction present in your patient’s cervical spine. It should be fairly easy for you to find some examples of these drawings elsewhere on the internet.

To reiterate this is a very valuable method for determining abnormal or pathological conditions such as hyper or hypomobility of the cervical spine.

Penning Method of Radiographic Determination of Loss of Range of Motion:

As an example if the normal for C2-C3 was 12.5 degrees and your patient was 20 degress, your percentage of normal would be 160%. If normal at C3-C4 was 18 degrees and your patient was 15 degrees your percentage of normal would be 83%. This is straight forward, if you would like all the normal values, these can easily be found on the internet.

The percentage of normal is figured out by dividing the patient’s values by Penning’s established normal values. For example C3-C4 15/18 = 83%. This essentially represents 83% of normal motion at the C3-C4 joint had been maintained. On the other hand you see that 17% of normal motion was lost when compared to the normal value as determined by Penning.

If you look at the C2-C3 level you see that you have 160% of normal motion, or 60% more than normal expected motion at this level. This is clear as to how to read this!

*Values above 100 are HYPERMOBILE

*Values below 100 are HYPOMOBILE

NORMAL at 100% (Values within 10% of 100 are considered to lie within the standard deviation of what is accepted as normal.) So anywhere between 90-110% for all intensive purposed is considered normal.

Further Analysis of example patient:

The interpretation of the above example is as follows; multiple areas of abnormal joint motion and dysfunction are present. Cervical Joint Hypomobility or a loss of normal joint range of motion was found at C3/C4, and C5/C6 motor units. Cervical Joint Hypermobility was present at the C2/C3 motor units.

You can also see a compensatory minimal Hypermobility present at C6/C7, but this falls within the normal standard of deviation.

Restated there are two areas of diminished joint range of motion and one with excessive motion involving the cervical spine segments during flexion/extension in the sagittal plane.

As is obvious, these abnormalities exist within the joints of the cervical spinal column and this analysis has been established radiographically.

As goes without saying, in addition to substantiating loss of joint range of motion, a Cape Girardeau Chiropractor should be looking to further evaluate possible existence of any neurological deficit (either sensory or motor). By simply looking at the mechanical and structural aspects of a particular case however, much can be objectively gained, and you will be able to stand with confidence in saying that structural integrity has improved on this patient if post care analysis shows improvement on Penning’s Method. Any degree of improvement substantiates improvement to the patient’s ligamentous integrity and of course increased normal range of motion. These two things alone could prove invaluable, should you find yourself testifying in court. Time for the “Retro Medicine” Revolution

OK so this is a term that I’ve made up! You won’t have heard of it before, because this is the first time it’s ever been mentioned. So I know that even though you are a smart, intelligent, thinking man or woman it’s OK for me to teach you what I mean by “Retro Medicine”.

For 13 years I’ve practised what most people might say is “traditional” medicine, having initially qualified as a traditional Chinese acupuncturist. That’s really an umbrella term for several modalities of treatment – needling (of course), acupressure massage, cupping, moxibustion, traditional Chinese dietary advice, lifestyle advice, relaxation techniques, exercise and body movement techniques. Over the years I’ve added to my training and qualifications to include Colon Hydrotherapy and Nutritional Therapy in my therapeutic portfolio, or tool bag. These both have ancient traditional roots too, and to appreciate their provenance think Hippocates as the “Father of Modern Medicine” who is quoted as famously saying “Let Food be Thy Medicine”; and think the Dead Sea Scrolls and the Egyptian hieroglyphics that depict and include instructions for inner cleansing of the bowel.

Recently I’ve been questioning whether or not what I do is truly “traditional”. Sure my therapies have traditional roots, but I certainly don’t practice any of them exactly as in ancient times. For example, long-necked gourds have been replaced by modern plastic disposable tubing and specula for colonics; I use modern turned stainless steel acupuncture needles; and my nutritional therapy practice, especially, is influenced hugely by modern clinical research. The recognition that what I do is essentially very modern, involving up to date practices but without losing their ancient essences and maintaining their traditional benefits, has led to my current reflections. Reflections that make me ask what is it that I really do? What is the thread that connects the various parts of my therapeutic practice? Simply, what is my “thing”?

I realise that I don’t want to turn back the hands of time. I don’t want to go back to ancient ways, and I don’t encourage others to do so either. But also, I can’t in good conscience agree with much of our modern western medical practices.

I’m not an enemy of modern medicine. There have been some amazing medical advances that deserve and have my respect. But… and there’s always a “BUT”… there is so much about modern medicine, particularly the use of pharmaceuticals, that is terribly damaging. I challenge you to name a drug that doesn’t come with a host of side effects – some mild, some terribly severe, with long lasting consequences far worse than the condition they aim to treat. Am I being over dramatic here? I don’t think so. There are so many examples littering the brief history of pharmaceutical intervention it’s shocking. Aspirin, once considered to be a benign, even safe, pain killer is now known to cause serious gastro-intestinal problems; hormone replacement therapy (HRT) originally considered to be the wonder drug that women could take forever was discredited just a decade later when it’s role in the development of breast and other hormonal cancers became apparent; antibiotics that have been prescribed with impunity over a few decades are now known to injure the immune system and are tolerated so well by the pathogens they aim to kill that we are faced with a modern epidemic of deadly antibiotic resistant strains of bacteria. What’s more, pharmaceuticals have no role in disease prevention – whatever their profit-hungry parent companies would suggest or have you believe!

So here’s my “thing” – I want the best of both worlds – the ancient and modern, the traditional and contemporary – for myself and my patients and for you too! In taking a retrospective look at what traditional and modern medicines and medicinal approaches have done for us we can cherry-pick what helps and what harms. “Retrospective”? Take a look in the dictionary and you’ll find it means something akin to looking backwards, contemplating what’s past. And “retro”? More about style – specifically of past times. And that’s it! My “thing” can be captured in two words – “retro medicine”.

“Retro Medicine” is my creation of a portfolio of clinical approaches for health and wellbeing resulting from understanding what works, what doesn’t, what in retrospect has been proven to help, to be effective, to be truly beneficial. Retro Medicine has no place for bias towards traditional or modern techniques. It has no place for prejudice against traditional or modern techniques. Rather, it will appeal to open-mindedness, critical thinking and reflection – the qualities associated with a genuinely intelligent mind. Retro Medicine is the pragmatic fusion of traditional and modern techniques that stand up to scrutiny and are shown clinically to benefit your health, promote well being, and help you to get the very best from your life.
